Dear Bach Flower Remedy client, as a pre-consultation requirement I’d ask you to read the Code of Practice. Then please complete the Disclaimer and Declaration. Code of Practice
- As a Bach Foundation Registered Practitioner (BFRP), I operate in accordance with the Bach Centre’s Code of Practice. The Code requires me to provide a secure environment in which the things we talk about will remain confidential.
- The Code also requires me to refuse cases that exceed my capacity or my area of professional competence, and also instructs me to refer those cases to more qualified professionals, so that clients can be most appropriately cared for. In particular, I reserve the right to contact your doctor or other qualified professional if I am concerned that you may be putting yourself at risk or becoming a risk to others.
- My role is to help educate you about Dr Bach’s system of remedies and help you use them to help yourself. Bach remedies aim to improve everyday emotional states. Taking them may in turn lead to other health benefits. They do not replace direct medical or psychological treatments. You as the client have sole responsibility for seeking qualified help for any medical conditions, symptoms or other issues that might require qualified intervention.
- If I do recommend you seek additional professional help, you take full responsibility for acting on that. If you have been receiving treatment from a medical or other professional I suggest that you inform them about your decision to use Bach remedies. Any decision to change any other treatments that you are receiving should be taken by you in conjunction with the associated professional.
- Fees charged by Linzi Martin are to cover the years she spent in learning, understanding and the personal experience to now be able to help you to learn and use the Bach system. If at the end of the consultation you choose to take the Bach remedies you may also choose to prepare a mixing bottle for yourself, or ask to have a bottle mixed for you. This is understood to be your personal choice and is not part of the service consultation offered.

I acknowledge that I am here on this and any subsequent visits, sessions, consultations and courses, either online or in person, solely on my own behalf and not on the behalf of anyone else.
I am not an agent for any government, federal, provincial, municipal, taxes or professional agency on a mission of entrapment or investigation.

Waiver
I agree to participate in the lifestyle and wellness services with the understanding that any life and lifestyle CMT, Listening Cheerleader, Bach Flower Remedy, Organising Clutter, Soothing Life Alchemy and any other treatment/care or educational information provided, are for my own health and lifestyle guidance, education and relaxation. That I use at my own discretion. I assume full responsibility for my choices and do so freely.
I hereby release, waive, acquit and forever discharge Linzi Martin, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts, errors or omissions by myself or by my CMT (Linzi Martin) as a result of the advice given by my CMT (Linzi Martin) or otherwise resulting from the CMT (Linzi Martin) relationship contemplated by this agreement. This agreement shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
Medical Disclaimer
I understand that Linzi Martin does not have medical credentials and she is not a licensed medical doctor or medical practitioner, chiropractor, psychologist, psychotherapist, osteopath, dietician or counselor.
I am not here for medical diagnostic or treatment procedures. I understand and agree that any services rendered by Linzi Martin are not designed to cure or prevent any disease, pain, deformity, injury, mental or physical condition of any kind.
If I have any health problem, health condition, or disease, I acknowledge that I will seek medical advise from a licensed doctor of medicine. I understand and accept that the services provided by Linzi Martin are for general wellness.
I understand that she cannot diagnose, prescribe, treat, cure or prevent any nutritional, medical or psychological disease, disorder, or condition. I understand Linzi Martin cannot advise, recommend, suggest or counsel me on any medical, dietary, emotional or psychological treatment, condition, disorder or disease.
I therefore take full responsibility and release Linzi Martin from liability with respect to any advice or treatment/care, which I may follow.
I fully understand that Linzi Martin does not offer allopathic drugs, surgery, chemical stimulants or any other conventional / allopathic treatments for my disease, condition, or illness, or perform any act that would constitute the practice of medicine for which a license is required.
I acknowledge that all services – life and lifestyle coaching and mentoring, CMT, Listening Cheerleader Sessions, Bach Flower Remedy and Soothing Life Alchemy and any other services and products – are not professional mental health care or medical care. They are not offered as a substitute for professional mental health care.
If I feel psychologically stressed to the point that is interfering with my ability to function, I will seek the help I need in the form of a professional counselor. I am not seeking crisis intervention. The work of life and lifestyle coaching / mentoring is meant to be done when major emotional and psychological wounds are already healing or healed.
I understand that life and lifestyle coaching / mentoring it is an unregulated industry and that Linzi Martin is not licensed in any jurisdiction.

Linzi Martin is registered as a Bach Foundation Registered Practitioner (BFRP) with the Bach Centre in the UK.
Coaching, Mentoring & Teaching – CMT Linzi Martin
While we hope that you will be encouraged by the resources offered on this site, please be aware that it is not a counseling service. Responses in email, by phone or any other means reflect the personal experience and beliefs of the CMT.
I understand and agree that I am fully responsible for my physical and emotional well-being during my CMT sessions, and subsequently, including my choices and decisions.
I understand that CMT is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care or substance abuse treatment, and I will not use it in place of any form of therapy.
I understand that to the extent our work together may involve lifestyle, career or business, my CMT is not promising any outcomes or success.
Fees & Cancellation
Fees: I agree to pay Linzi Martin for services and products, the full amount plus any processing fees on time. Cheque / Check – In the event this is ‘insufficient funds’ I agree to pay full restitution of costs.
I understand that all fees are non-refundable.
Disputes: I agree to bring any dispute to Linzi Martin’s attention as soon as possible and without undue delay. Linzi Martin agrees to make every effort to resolve any dispute with client’s in a timely manner.
Cancellation: 48 hours notice in writing is required for cancellations. A late cancellation fee of half of the amount of the session (service) will apply. In case of emergency cancellations to email linzi2simplifylife@gmail.com.
Governing Law & Arbitration
I understand that the resolution of any and all disputes arising from or in connection with my relationship with Linzi Martin, including any disputes arising under or in connection with this Agreement, shall be governed by and construed in accordance with the laws of the province British Columbia (other than conflict of laws rules) and the laws of Canada applicable therein. For disputes between you and Linzi Martin / NaturallyLinzi.com that results in litigation you will have to submit to the jurisdiction of a Canadian court.

Client Confidentiality
I understand that my identity and any information about me will be held in confidence, except when there is a serious perceived or actual threat to the safety of yourself, another person including child and elder abuse or threaten to harm Linzi Martin. I understand that necessary actions will be taken and my confidentiality agreement is limited in this capacity. If Linzi Martin is ordered by a court or police to provide information or to testify she will do so.
I understand that the use of technology is not always secure and I accept the risks of confidentiality in the use of email, text, phone, messaging applications, online conferencing and other technology.
We may amend this Privacy Policy & Client Confidentiality from time to time without notice to you, in which case, we will publish the amended version on the Website.
